What is a Closed Lock?

A closed lock is clinically diagnosed as a Disc Displacement Without Reduction (DDWOR). In a healthy joint, the mandibular condyle smoothly interacts with the disc, facilitating jaw movement.
The disc is supported by connective tissue that helps maintain its position and prevents excessive forward movement of the disc. This condition is clinically diagnosed as a Disc Displacement With Reduction, and in most cases is not painful nor does it impede on normal functioning of the jaw.
A very small percentage of people who are experiencing this disc clicking can progress to a DDWOR, where the disc obstructs the condyle from moving enough to allow the jaw to open fully. This condition may cause pain, stiffness, and a noticeable deflection of the jaw during movement. Physical therapy plays a crucial role in managing acute DDWOR by utilizing muscle release techniques, trigger point dry needling, and joint mobilizations to alleviate symptoms and restore functional movement.
The TMJ is remarkably adaptable and tissue remodeling can occur to create a "pseudo-disc" that facilitates smooth joint motion.
